New Cancer Guidelines Integrate Immunotherapy in Algerian Care

Algeria's 2026 Guides Thérapeutiques en Oncologie Médicale (Algerian Therapeutic Guides in Medical Oncology) mark a significant evolution in the management of high-risk triple-negative breast cancer (TNBC). These updated national guidelines now incorporate immunotherapy and germline BRCA-directed treatment, moving beyond traditional chemotherapy-centric approaches. For patients with stage III disease, the recommended standard involves neoadjuvant chemotherapy combined with pembrolizumab, reflecting a modern approach to cancer care.

The pathological response achieved after neoadjuvant treatment now plays a crucial role in determining subsequent therapeutic strategies. Patients who attain a pathological complete response (pCR) following neoadjuvant chemo-immunotherapy, for instance, do not receive additional systemic adjuvant treatment in this specific pathway. Furthermore, the guidelines emphasize considering germline BRCA1/2 testing, especially when results can influence treatment decisions. For eligible high-risk patients with a BRCA1/2 mutation, one year of adjuvant olaparib is specifically included in the 2026 guideline.

This shift necessitates a more integrated approach to treatment planning for Algerian patients with TNBC, requiring coordinated access to pathology, biomarker assessment, genetic testing, surgery, radiotherapy, and systemic therapy. Treatment decisions, made through multidisciplinary discussions, now factor in the tumor biology, response to neoadjuvant therapy, and the presence of actionable germline alterations, alongside traditional considerations. The significant evolution in these guidelines highlights Algeria's commitment to advancing patient care, with the next focus on ensuring consistent nationwide implementation of these sophisticated new recommendations.
